MOCC offers 41 skilled nursing beds to provide long term care with services. We have a spa like whirlpool room and private or semi-private rooms. We also have several providers that round in the facility. Medicare/Medicaid certified.

Rehab
We offer skilled nursing, PT, OT, and ST for your short term rehab stay. Private and Semi-Private rooms available.
Assisted Living
We offer assisted living services with Private and Semi-Private rooms available. If you need more assistance and transition to nursing home you do not have to change rooms or facilities.
Respite Care
We can provide a short term home for your loved one while regular care givers take a trip, get medical attention, or just take a well earned break.
Hospice
Hospice services offered with the hospice company of your choice.
Outpatient Therapy
We offer outpatient therapy. Please call for more information on therapy availability.
Meals on Wheels
We offer Meals on Wheels for community members. Call for more information.
